For observations in patients at risk for HCC. Settle the gates first (LR-NC, LR-1/LR-2, LR-TIV, LR-M); otherwise set the size, arterial enhancement, and additional features.

First, settle these — select if it applies; any one of these decides the category on its own

LR-NC: image degradation or an omitted phase precludes categorization. LR-1: definitely benign — simple cyst, typical haemangioma, definite perfusion alteration, focal fat or sparing, confluent fibrosis. LR-2: probably benign — the same entities short of certainty (probable haemangioma, probable perfusion alteration), or a <20 mm distinctive nodule with no major, LR-M or malignant ancillary feature.

Arterial phase hyperenhancement (APHE)

Only nonrim APHE feeds the LR-3/4/5 table. Rim APHE is a targetoid LR-M feature — pick “Rim APHE” rather than “Nonrim APHE present”.
