PECARN — Pediatric Head Injury

For children with GCS 14–15 after blunt head trauma within 24 h. Identifies who is at very low risk of clinically-important TBI (ciTBI).

High-risk predictors — CT recommended if any

Intermediate predictors — observation vs CT if any

Applies only to children with GCS 14–15 evaluated within 24 h of blunt head trauma; not for trivial mechanisms, penetrating trauma, known brain tumors, or pre-existing neurologic disorders. A higher-risk or worsening child warrants CT regardless. Decision support only — not a substitute for clinical judgment.
